LIABILITY WAIVER
I, the member, acknowledge that participation in rebound exercise activities at Revive Rebound Studio might involve inherent risks, including but not limited to, falls, muscle strains, joint injuries, cardiovascular incidents, or other physical injuries. I voluntarily assume all such risks associated with my participation. It is hereby understood that the selection of exercise programs and methods shall be my, the member`s, entire responsibility. Revive Rebound Studio shall not be liable to me, family, executors, administrators, personal representatives or associates for any claims, demands, injuries, damages, or actions arising due to injury to myself or property arising out of or in connection with the use of the services, facilities or the premises. I hereby release, waive, and discharge Revive Rebound Studio, its owners, employees, trainers, independent contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of any injury, damage, or loss sustained by me while participating in activities at the facility, whether caused by negligence or otherwise.
I confirm that I am in good physical condition and have no medical condition that would prevent me from safely participating in rebound exercises. I acknowledge that I have been advised to consult with a physician before engaging in any physical activity and that I assume full responsibility for my participation. Revive Rebound Studio shall not be liable for any injury arising out of my, the member`s, disability impairment or ailment preventing me from engaging in active or passive exercise, or that would be detrimental or adverse to my health, safety or physical condition if I do engage or participate. I, the member, am aware of my medical history and should consult with a physician prior to engaging in exercise or continuing to exercise if a medical condition appears or appears to be developing.
I agree to indemnify and hold harmless Revive Rebound Studio, its owners, employees, trainers, and affiliates from any claims, damages, liabilities, or expenses (including legal fees) that may arise as a result of my participation in activities at the studio.
I grant Revive Rebound Studio permission to use my photograph, video, or likeness for promotional and marketing purposes, including social media and advertising.
I have read and fully understand this Personal Liability Waiver and Release Section. I acknowledge that I am signing this document voluntarily and that by doing so, I am waiving certain legal rights. This section will survive any cancellation of this Agreement.
